If you're a new client, please complete the following forms.  The client intake form should be filled out ahead of time and brought to your first session.  You will not be able to fill it out at my Watsonville (Sunset State Beach) office, as I do not have a waiting room there (but do have one in my Soquel office).   At the end of your first session I will have additional comprehensive mood surveys to give to you to be filled out prior to your second session.
